Ok, so I know that orange items can be obtained from bosses and chests. I also have found them in vending machines and slot machines, how about other sources? Is it possible to get a legendary out of a cardboard box? or a skag pile? …or one of those flying dactyls? Is there an actual chance however microscopic?
How about normal mode vs TVHM or UVHM? I mean, does that chance exists in UVHM OR TVHM but not in normal mode? or only UVHM, or none? or all but even more minuscule?
It is possible to get a Legendary (it’s even possible to score a Pearlescent) out of stuff like Skag piles and garbage bins, but it is extremely low. It has happened to me a few times.
It can happen on all game modes and I think the chance is increased on TVHM and UVHM, but I am not entirely sure on that.

I’ve gotten orange out of skag piles on quite a few occasions and rak or dactyl hanging goo in wildlife preservation but I use the starting out “vault hunters relic” with higher chance of rare loot drop, ( does that even help…not sure).
You can get legendary items from any loot source but they have a higher chance of dropping from their assigned loot pinata, midgets, raid bosses, tubbies, and the loot train in Marcus’ Mercenary Day HH pack.
It only boots the chance of you getting something better than a white, such as a green, by a smidge. It is a throw away item once you get another relic.
Everywhere, but have increased drop options from the bosses.

Yep. Both Moxxi’s and Tina’s slot machines have a small chance to give you a legendary. For Moxxi’s, you need three vault symbols. For Tina’s, you need to roll a 20 on the dice.
I have yet to pull a Legendary from Butt Stallion or the grinder at the Keep, but I have faith that it’ll happen!
