I had accumulated 400 moonstones and since I like completionism I decided to blow them for this quest on NVHM and TVHM to get it off my list. NVHM I pay, open the chest, get glitch loot. Then I move to TVHM, pay the price, open the chest and… it’s empty.
Not that I would have needed anything out of it at that level but seriously now Is this quest a one time reward / per character and then you get nothing for your 200 moonstones on different difficulties or did I just fall victim to some random glitch? I’d like to know since it’s one of the things I can blow my moonstones for at level 70 campaign resets but there’s no way I’m dropping 200 in there in UVHM ever if it’s going to just be empty.

The wiki says once per mode, for what it’s worth. Unless someone drops in with more info, I’d suggest filing a support ticket to see if it’s a known glitch.
as @VaultHunter101 says, I believe it’s once per mode, but have yet to ck it out, as I have never even did that mission once yet in any of my multiple toons, I need it for the 100%, but just hate to drop that many moonstones.
I tend to just farm the chest claptrap opens just after the holding cell for all of my Glitch weapons
